Some more information is needed - what is your occupation? how noisy is your work environment? if you work in an office, do you always wear headphones to listen to music while you work? do you regularly go to clubs or concerts with amplified music? what type of music do you listen to? etc

I did play drums a bit in my teens and have been out nightclubbing and concerts over the years but nothing more than the average young person I’d guess. I do wear earphones to play music but I’m always quite careful to ensure that they don’t go over the limit.

Looks like normal-ish presbyacusis (nerve hair-cell decline). Might be a little noise damage added to the mix or congenital issues to explain the inverted curve in the upper HF.
Aside from that; the only thing I’d be bothered about is the speed/stability of the decline over the last few years. Perhaps ask your hearing provider to put you on 6-monthly tests (at least for a year or so) to see if what’s going on too.

Um_bongo’s post will be accurate so please go with his advice. You are unlucky to lose your high frequency hearing at your age so the cause looks to be genetic.
The shape of your hearing loss with frequency occurs in lots of people who have lost hearing, so hearing aids are easily found that will help a lot.

Here I will disagree a little bit To use there needs to be more age to use this term. This looks to be noise induced and and genetics can definitely be a part of this loss as for some it makes them more susceptible to hearing loss than others from noise. As far as the head phone use questions of what type as if in the canal it can cause damage if there is no outlet so there can be an escape of pressure. If no outlet then even and comfortable/safe limits damage can occur. I would recommend hearing aids and then for then next couple years have testing done to see if it is continuing JMHO and 38 yrs of practic.
