Is there any way that the audiologist can see that the settings have been modified (even we I revert to the previous configuration before an appointment with the audiologist)?

This happens as soon as another PC (in this case the audiology clinic that set your up) is used, target gives a little pop up to let them of a data mismatch etc

However, there is a significant hiss in quiet situations that I am very keen to get rid of.
This may be solved by “Soft Noise Reduction” in the Calm program if you mean very soft humming of fans or refrigerators.

I bought my aids from wholesale hearing and they have online audiology support. They couldn’t connect to me through the app because my husband had been using target. Thankfully I defaulted back to him adjusting them which was fine. Told them I no longer needed a remote session ! Saved £1500 against the price my original audiologist was going to charge me and he didn’t even know how to set them up!
