What is the connection you’re using from the tv to the sound bar? Or from any set box box (STB) to the sound bar?
What is the exact model name/number of the tv so I can find a manual to see what it has?

Sounds fine to me. So what’s the problem?
Are you getting sound to the aids and sound out of the sound bar at the same time?
If you mute the tv sound does the sound get muted to the aids as well?
Do you turn the volume down on the tv but you can still hear through the aids?

LG TV doesn’t support two digital audio outputs at the same time. You need to buy a HDMI ARC extractor and put it before the soundbar and use optical out from the extractor to your TV Connector. Something like this one:
Hdmi Arc Audio Extractor Passthrough I Avantree HAX0670
Connect your TV audio effortlessly with HDMI ARC, Optical, or RCA outputs. Supports bypass to soundbar and delivers Dolby 5.1 surround for full compatibility.

Thank you @Spindel . I was heading there depending on the answers to my questions.
@glennwithbothns : What is the make/model number for the sound bar? I wonder if it has outputs.

I had an old LG smart TV with Yamaha soundbar and Phonak TV streamer, and I managed to make everything work together using an optical signal splitter (both the soundbar and the streamer have optical input, but the soundbar has no HDMI input).
The optical splitter (1 input → 2 output) is quite cheap (15€) and the induced signal power loss (1/2 split) doesn’t seem to be a problem for such short optical fibers.
The LG TV stopped working 1 year ago and I replaced with a Samsung, keeping the same identical setup.

A second choice would be to get an optical splitter and two extra fibers. It’s simple and pretty much plug and play. Mine was working within a few minutes. This is only useful if the soundbar works with an optical input.

@glennwithbothns Just following up here to see if you’ve resolved your issue with getting audio from your TV Connector and your soundbar simultaneously. I’m from Avantree and as @Spindel suggested our HDMI ARC extractor (HAX-06) should be able to help in your situation.
As a general rule, most TVs do not inherently support simultaneous audio output from both HDMI and the optical port. You usually have to select one or the other as the primary audio output in the TV’s settings menu.
Please let me know if you would like any advice or help with the HAX-06 (or any other part of your setup).
Thanks!

Just went through this recently. Drove me nuts until my GS showed the simple way.
I have an LG C5 and an LG soundbar along with the Oticon tv streamer. He hooked one optical cable from the tv to the IN connection on the tv adapter and then he hooked another optical cable from the OUT connector on the tv adapter to the soundbar. We have DTV streaming so he hooke it up to HDMI 1. Everything works perfect now.
