Help entering Costco hearing test into my profile

I am having trouble interpreting this print out of my hearing test from Costco. I have entered what I could understand of it in my profile, but much of this print out is inadequately labeled, relative to the fields provided in the “My Hearing Tests” section of this website.I would love to get any feedback on this. I have been wearing hearing aids for decades now, from to my current ReSound GN’s. I am having serious trouble understanding what is being spoken, although I hear the volume well enough.

You did fine entering the Hearing Loss Thresholds part, which is the only required and important part.

Optional: Bone Conduction Results are not important in your case; don’t worry about those results.

Similarly with the Optional: Word Recognition Score results.
